Vitamin D in Pregnancy: Why So Many Australian Women Are Deficient

Vitamin D deficiency during pregnancy is more common in Australia than most people expect. Here's why sun exposure alone isn't enough, what the consequences of deficiency are, and how to address it.

Australia is one of the sunniest countries on earth. It seems reasonable to assume that Australians, exposed to more UV radiation than most populations on the planet, would have excellent vitamin D levels. The reality is more complicated. Vitamin D deficiency is common in Australia, affects a significant proportion of pregnant women, and has specific consequences for both the mother and the developing baby that make it worth understanding and addressing.

This is not a niche nutritional concern. The Royal Australian and New Zealand College of Obstetricians and Gynaecologists identifies vitamin D deficiency as one of the more frequently identified nutritional issues in pregnancy, and Australian research suggests rates of deficiency in pregnant women ranging from 20 to over 60 percent depending on the population studied and the season.


Why vitamin D deficiency is so common in Australia

The counterintuitive nature of widespread vitamin D deficiency in a sun-drenched country is best explained by three overlapping factors.

Sun protection behaviour. Australian public health campaigns have, for decades, successfully communicated the importance of sun protection — slip, slop, slap, seek, and slide. Australians are among the most sun-aware populations in the world, and many consistently avoid sun exposure during peak UV hours, cover up, and apply sunscreen. This behaviour is entirely appropriate for skin cancer prevention. It also significantly reduces the UV-B exposure that the skin requires to synthesise vitamin D.

Modern indoor lifestyles. Office work, air-conditioned environments, and the shift away from outdoor physical activity mean that many Australians spend the majority of their daylight hours indoors. The sun cannot produce vitamin D through a window — glass filters the UV-B wavelengths required for synthesis. Time spent in a car, in an office, or at home does not contribute to vitamin D production regardless of how bright the day is outside.

Geographic and seasonal variation. While this is less relevant in tropical and subtropical Australia, women in southern states including Victoria, Tasmania, and parts of South Australia experience genuinely low UV-B levels during winter months. In these regions, adequate vitamin D synthesis from sun exposure alone is simply not achievable between May and August regardless of outdoor time.

Skin pigmentation. Melanin, the pigment that gives skin its colour, reduces UV-B penetration and therefore reduces vitamin D synthesis for the same amount of sun exposure. Women with darker skin pigmentation require significantly longer sun exposure to produce the same amount of vitamin D as women with lighter skin, which means that in the Australian context — where sun exposure is often limited by protective behaviour — darker-skinned women are at considerably higher risk of deficiency.

Dietary sources are limited. Unlike some nutrients, vitamin D is found in relatively few foods and in quantities that make it difficult to meet requirements through diet alone. The primary dietary sources — fatty fish, egg yolks, and fortified foods — provide only modest amounts. There is no practical way to meet pregnancy vitamin D requirements through food alone without supplementation.


What vitamin D does during pregnancy

Vitamin D has several important functions during pregnancy that extend beyond the bone health role most people associate with it.

Calcium absorption. Vitamin D is essential for the absorption of calcium from food. Without adequate vitamin D, even a high-calcium diet does not translate to adequate calcium availability for your baby's bone and tooth development or your own skeletal maintenance. The two nutrients are functionally interdependent — calcium intake without vitamin D is significantly less effective.

Fetal bone and skeletal development. Your baby's skeleton mineralises progressively across the second and third trimesters. Adequate vitamin D is required for this process to occur normally. Severe deficiency during pregnancy is associated with neonatal rickets — a condition involving soft, malformed bones in the newborn — though this is most common in settings with extreme deficiency rather than the moderate deficiency more typical in Australia.

Immune function. Vitamin D plays a significant role in immune regulation for both mother and baby. Research has associated vitamin D deficiency during pregnancy with increased risk of gestational diabetes, pre-eclampsia, and preterm birth, though the evidence for these associations is stronger for some outcomes than others. The mechanisms are not fully understood but involve vitamin D's role in inflammatory regulation and placental function.

Newborn vitamin D stores. Your baby is born with the vitamin D stores you have built during pregnancy. Breast milk is low in vitamin D regardless of maternal status, which means a newborn whose mother was deficient during pregnancy arrives with limited stores and receives limited vitamin D through feeding. Australian guidelines recommend vitamin D supplementation for breastfed infants whose mothers were vitamin D deficient during pregnancy — your GP or child health nurse can advise on the appropriate dose.


How vitamin D status is assessed

Vitamin D status is measured through a blood test that checks your serum 25-hydroxyvitamin D level, abbreviated as 25(OH)D. The result is reported in nanomoles per litre (nmol/L).

In Australia, the general thresholds are: below 25 nmol/L is considered severe deficiency, 25 to 49 nmol/L is deficiency, 50 to 74 nmol/L is inadequacy, and 75 nmol/L or above is considered sufficient. Some practitioners and guidelines suggest levels above 100 nmol/L are optimal during pregnancy, though there is not full consensus on the upper threshold.

Vitamin D is not always included in the routine antenatal blood tests ordered at your booking appointment. You may need to specifically request it. If you have risk factors for deficiency — including darker skin pigmentation, limited sun exposure, wearing covering clothing, or living in southern Australia during winter — asking for vitamin D to be checked at your first appointment is reasonable and worthwhile.


How much vitamin D you need during pregnancy

The Australian NHMRC recommends an adequate intake of 5 micrograms (200 IU) of vitamin D per day during pregnancy, with an upper limit of 80 micrograms (3200 IU) per day. However many practitioners and researchers consider these recommendations conservative relative to the evidence, and many GPs recommend higher supplemental doses for women with confirmed deficiency.

The appropriate dose of vitamin D supplementation during pregnancy depends on your baseline blood level, your risk factors, and your sun exposure. A woman with confirmed deficiency in winter in Melbourne requires a different dose than a woman with adequate levels in subtropical Queensland. This is a conversation for your GP rather than a one-size-fits-all recommendation.

What is clear is that the standard dose of vitamin D included in most Australian pregnancy multivitamins — typically 200 to 400 IU — is unlikely to be sufficient to correct established deficiency. If your blood test shows deficiency, a higher therapeutic dose prescribed by your GP is required alongside your regular multivitamin.


Safe sun exposure during pregnancy

Sensible sun exposure remains the most natural and effective way to maintain vitamin D levels, and it is not incompatible with sun safety when approached appropriately.

In most parts of Australia, brief sun exposure to the arms and legs during the morning or afternoon — outside of the peak UV hours of 10am to 2pm — provides meaningful vitamin D synthesis without the skin cancer risk of midday exposure. The amount of time required varies significantly by skin type, season, and location. The SunSmart UV Alert, available through the Bureau of Meteorology and the Cancer Council Australia, indicates when UV levels are sufficient for vitamin D synthesis in your area on any given day.

In southern Australia during winter, UV levels may not be sufficient for vitamin D synthesis even with extended outdoor exposure. In these circumstances, supplementation is not a lifestyle choice but a nutritional necessity.

During pregnancy, extreme heat and dehydration are risks associated with outdoor activity, particularly in the Australian summer. Morning sun exposure rather than midday or afternoon is appropriate.


Vitamin D supplementation during pregnancy

For most pregnant women in Australia, vitamin D supplementation is appropriate and worth discussing with your GP.

Most pregnancy multivitamins contain a small amount of vitamin D. Whether this is sufficient depends on your baseline levels and your sun exposure. If you have confirmed deficiency, a separate vitamin D supplement at a therapeutically appropriate dose is likely needed alongside your multivitamin.

Vitamin D3 (cholecalciferol) is the preferred supplemental form as it is more effective at raising blood levels than vitamin D2 (ergocalciferol). Both are available at Australian chemists without prescription up to certain doses. Higher therapeutic doses require a prescription.

Vitamin D toxicity is possible with excessive supplementation, though it requires very high doses over extended periods. Standard supplementation at doses recommended by your GP is safe during pregnancy. Do not self-prescribe very high doses without medical guidance.
